Responsibilities:
- Provide skilled nursing care to patients in their homes, including but not limited to administering medications, performing wound care, and managing chronic conditions.
- Conduct comprehensive assessments of patients' health status and develop personalized care plans to address their specific needs.
- Monitor patients' conditions and report any changes or concerns to the healthcare team.
- Collaborate with other healthcare professionals, such as occupational therapists, physical therapists, and social workers, to ensure patients receive comprehensive care.
- Educate patients and their families on self-care techniques, medication management, and disease prevention.
- Utilize electronic medical records (EMRs) such as Epic to document patient care and communicate with the healthcare team.
- Perform IV insertion and manage acute pain management.
- Transport patients to and from medical appointments, including critical care transports, when necessary.
- Provide care to patients with memory care needs and developmental disabilities.
- Conduct laboratory tests and interpret laboratory results as needed.
- Manage patients with neonatal ventilator care needs.
- Demonstrate a commitment to ongoing learning and professional development.
Requirements:
- Current licensure as a Registered Nurse in the state where the position is located.
- Experience in home health or a related field.
- Strong knowledge of internal medicine and chronic disease management.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Proficiency with electronic medical records (EMRs) such as Epic.
- Ability to lift and move patients as necessary.
