What are the responsibilities and job description for the Licensed Practical Nurse position at Vincentian Collaborative System?
Summary:
According to specific assignment and in accordance with established standards, the Licensed Practical Nurse provides technical nursing care, assists with resident care planning and observation of resident's condition, and reinforces physician, resident and family instructions.

Essential Duties and Responsibilities:

4. Adheres to all facility and department work rules, policies and procedures.
5. Performs individualized personal care for residents in accordance with the established nursing care plan and within scope of licensure.
6. Collects data, reports and records observation of residents' condition.
7. Assists physicians with procedures and examinations.
8. Assists with admitting, discharging, transporting and transferring residents.
9. Participates in resident care conferences.
10. Performs service duties and specific assignments such as cleaning instruments and equipment, collecting specimens, cleaning work areas and replacing supplies.
11. Attends required continuing education programs and unit staff meetings.
12. Assists in the orientation of new personnel.
13. Maintains confidentiality of resident and staff information.
14. Anticipates and takes action to maintain a safe environment. Incorporates safety/risk management standards into daily work practices.
15. Performs job functions in a manner that promotes efficient use of resources.
16. Assumes charge of unit in absence of Registered Nurse, when assigned.
17. Assumes responsibility, in the absence of Registered Nurse or when directed by Registered Nurse, for duties such as:
18. Passing medications
19. Administering injections and treatments
20. Making rounds with physician and assisting in examining and treating residents
21. Assists in developing nursing care plans
22. Admits and orients new residents
23. Reviews physician's orders, transfer and pre-admission information for additions, omissions, discrepancies, etc.
24. Reviews the resident's medical and social history25. Requisitions and arranges for diagnostic and therapeutic services as ordered by resident's physician within the facility or outside facilities
26. Orders prescribed medications, requisitions supplies and equipment
27. Notifies dietary department of new orders and changes in resident diets
28. Assembles residents charts, assures all permits, authorizations and releases and other required forms are attached
29. Transcribes physician's orders to resident's medication and treatment record, etc.
30. Maintains narcotic records, unit records, and charting duties
31. Assists with supervision and care assignments of nursing personnel
32. Notifies physician of changes in resident's condition and any unusual or abnormal observations33. Performs all other duties assigned by supervisor.
34. Required to support the organization's Code of Ethical Conduct and Compliance Program by timely reporting any actual or suspected compliance, fraud and abuse, ethics or policy violations.
Education/Experience:
1. Current licensure to practice practical nursing in the Commonwealth of Pennsylvania in good standing or a Temporary Practice Permit required.
2. Appropriate clinical knowledge of nursing care, significant signs and symptoms of disease, sterile and aseptic techniques, nursing procedures and treatment, and developed skills to provide technical nursing care required.
3. Current CPR Certification preferred.
4. Previous experience as a Licensed Practical Nurse in long term care preferred.
