Virtus Healthcare Staffing is seeking a dedicated Board Eligible/Board Certified Family Medicine Physician to provide primary care services at our partner, a 160-bed skilled nursing facility (SNF) in Waterloo, NY. This is an excellent opportunity for a physician interested in focusing on the care of geriatric patients within a long-term care setting.
As the Primary Care Physician for residents, you will be responsible for providing comprehensive medical care, managing chronic conditions, and collaborating with the facility's interdisciplinary team to ensure high-quality, resident-centered care.
Qualifications
Required:
Medical Degree (MD or DO) from an accredited institution.
Completion of an accredited Family Medicine residency program.
Board Eligible or Board Certified (BE/BC) in Family Medicine.
Eligible for or possess an active, unrestricted New York State medical license and DEA registration.
Preferred:
Experience or demonstrated interest in caring for patients within a Skilled Nursing Facility (SNF) or geriatric setting.
Geriatric certification or experience is a plus.
Key Responsibilities
Patient Care & Clinical Excellence
Provide ongoing primary medical care to residents within the skilled nursing facility.
Conduct comprehensive geriatric assessments, including history taking and physical examinations.
Diagnose and manage acute and chronic medical conditions common in the elderly population.
Develop, implement, and regularly review individualized care plans for residents.
Prescribe appropriate medications and treatments, monitoring for effectiveness and side effects.
Collaboration & Compliance
Collaborate with nursing staff, therapists (PT/OT/SLP), social workers, dietitians, and consulting specialists to provide coordinated care.
Participate in care planning meetings and communicate effectively with residents and their families.
Maintain accurate and timely documentation in the facility's medical record system.
Adhere to all facility policies and relevant state/federal regulations for long-term care.
