Physical Therapist

Purpose of Position:

Plans and administers physical therapy treatment programs for medically referred patients to restore function, relieve pain and prevent disability following disease, injury or loss of body part through a variety of treatment modalities.

Responsibilities:

1. Assesses patients for functional limitations or needs related to diagnosis.
2. Develops a therapy plan of treatment with specific goals in collaboration with the patient, family, physician and other health team members.
3. Provides direct care to treat the medical/surgical condition through the use of accepted physical therapy modalities.
4. Instructs and supervises the patient/family and home health aide (and others) in the home exercise program and other aspects of patient care as appropriate and documents their response to instruction.
5. Provides written home exercise program to patient (and others) as needed and also provides a copy for the patient’s record.
6. Continually re-evaluates the patient's condition and modifies the plan of treatment as appropriate.
7. Identifies and reports significant clinical information to the physician, Director of Rehabilitation, Rehab Supervisor, primary nurse coordinating services and other disciplines.. 
8. Documents all clinical evaluations, treatments and patient's response to therapy interventions on a timely basis.
9. Participates in patient care conferences for coordination purposes and documents outcomes.
10. Participates in quality improvement activities as appropriate.
11. Recommends adaptive equipment and assistive devices to maximize patient function.
12. Provides education and consultation with patients/families and agency staff as needed.
13. Provides case management activities for patients/families when appropriate, under the direction of Rehabilitation Department Supervisor
14. Promotes and facilitates optimal functioning and independence.
15. Systematically plans for discharge and continually evaluates the patient’s progress toward this goal.
16. Provides physical therapy only evaluations as directed.
17. Submits completed clinical documentation in accordance with agency policies.

Qualifications:

1. NYS Professional License and current registration to practice as a Physical Therapist.
2. One (1) year of full time experience in direct provision of physical therapy service.            
3. Home care experience preferred.

1. Automobile in good working order and auto insurance in the amount specified by VNSW.
2. Valid driver’s license.
3. Computer Knowledge preferred.

1. Knowledge of McKesson Clinical Software preferred.
2. Must maintain CEU requirements.
3. Professional Liability Insurance in the amount specified by VNSW
