Purpose of Position:
Plans and administers physical therapy treatment programs for medically referred patients to restore function, relieve pain and prevent disability following disease, injury or loss of body part through a variety of treatment modalities.
Responsibilities:
- Assesses patients for functional limitations or needs related to diagnosis.
- Develops a therapy plan of treatment with specific goals in collaboration with the patient, family, physician and other health team members.
- Provides direct care to treat the medical/surgical condition through the use of accepted physical therapy modalities.
- Instructs and supervises the patient/family and home health aide (and others) in the home exercise program and other aspects of patient care as appropriate and documents their response to instruction.
- Provides written home exercise program to patient (and others) as needed and also provides a copy for the patient’s record.
- Continually re-evaluates the patient's condition and modifies the plan of treatment as appropriate.
- Identifies and reports significant clinical information to the physician, Director of Rehabilitation, Rehab Supervisor, primary nurse coordinating services and other disciplines..
- Documents all clinical evaluations, treatments and patient's response to therapy interventions on a timely basis.
- Participates in patient care conferences for coordination purposes and documents outcomes.
- Participates in quality improvement activities as appropriate.
- Recommends adaptive equipment and assistive devices to maximize patient function.
- Provides education and consultation with patients/families and agency staff as needed.
- Provides case management activities for patients/families when appropriate, under the direction of Rehabilitation Department Supervisor
- Promotes and facilitates optimal functioning and independence.
- Systematically plans for discharge and continually evaluates the patient’s progress toward this goal.
- Provides physical therapy only evaluations as directed.
- Submits completed clinical documentation in accordance with agency policies.
Qualifications:
- NYS Professional License and current registration to practice as a Physical Therapist.
- One (1) year of full time experience in direct provision of physical therapy service.
- Home care experience preferred.
- Automobile in good working order and auto insurance in the amount specified by VNSW.
- Valid driver’s license.
- Computer Knowledge preferred.
- Knowledge of McKesson Clinical Software preferred.
- Must maintain CEU requirements.
- Professional Liability Insurance in the amount specified by VNSW
