Pay: $91,000-$99,000/annually

About the job:

 The Clinical Care Manager under the supervision of the Center Director oversees and manages the Senior Community Care (SCC) clinic, home care and medication departments to ensure the efficiency, consistency, continuity, and quality of care provided to program participants. Is accountable for meeting organizational and participant outcome and utilization goals.

 

Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Required:

  • Unencumbered license as a Registered Nurse (RN) in the State employed
  • Bachelor’s or Master’s Degree from an accredited school in nursing strongly preferred.
  • A minimum of five (5) years of nurse management experience in a hospital, nursing home or community-based setting strongly preferred
  • Proven experience leading a team of professionals. A minimum of one year’s experience in working with the frail or elderly
  • Must have a valid driver’s license, proof of insurance and have means of
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • A minimum of one year’s experience in working with the frail or elderly population

 

Essentials: 

  • Provides oversight of the overall effectiveness of clinical care provided to SCC participants across all care settings and is responsible to meet defined organizational participant outcome, utilization, and quality goals.
  • Provides oversight, management, and coordination of the consistency and quality of the twenty- four (24) hour care provided to program participants across all care settings.
  • Provides oversight, management, and direction of the overall operational effectiveness of PACE clinic, home care and medication departments and is responsible for effectuating and maintaining effective and efficient operational systems, processes, and workflows in compliance with applicable regulations and organizational policies to meet participant care needs and organizational objectives. This includes, but is not limited to, staffing schedules, inventory management and procurement, policies and procedures, clinical workflows,
  • Provides direct supervision and support of the clinic nurses, clinic secretary, home care supervisor, and medication department staff.
  • Responsible, in coordination with the clinical team and IDT, for utilization management including minimizing unnecessary or preventable utilization, safe and effective discharges and transitions between care settings, and utilization review and improvement efforts to meet participant care needs.
  • Responsible for oversight and management of SCCCO Medication Department. Ensures the organizational policies and procedures are strictly adhered to. Works in collaboration with local, regional, and VOANS quality and compliance personnel ensure effective and compliant med room operation and to prevent, detect, and correct regulatory non-compliance and fraud, waste, and abuse.
  • Responsible for receiving, documenting, reporting, and following-up on all medication events and errors occurring through pharmacy, participant administration, and direct administration by Senior Community Care staff. Participants in other investigations as required or requested.
  • In conjunction with the Integrated Care Managers, collaborates with contracted providers and facilities for the coordination of effective care and treatment of program participants
