Warm Hearth at Home is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team on a FT basis. The LPN will deliver high-quality nursing care to patients in their homes, working under the supervision of a Registered Nurse and in accordance with the plan of care, state licensure guidelines, and organizational standards.
We're looking for someone with strong critical thinking skills, a hospital or home health background, and excellent assessment skills. Someone with strong communication skills, the ability to time management, and someone that adapts well to change.
Responsibilities
By providing companion and caregiver services in a client’s home, Warm Hearth at Home helps families develop a reliable support network and can offer viable solutions to everyday problems. Warm Hearth at Home offers the additional support our clients need to maintain their current living arrangement or navigate the transition to a higher level of care when needed. Service options include care management, medication management, skilled nursing, rehabilitative therapies and home health aides.

Responsibilities
- Provide skilled nursing care to patients in their homes as directed by the RN and physician orders
- Perform clinical procedures such as wound care, catheter care, medication administration, and monitoring vital signs
- Document timely, accurate, and complete patient information in the electronic medical record
- Collaborate with interdisciplinary team members to ensure coordinated care and positive outcomes
- Communicate effectively with patients, families, and caregivers to educate and provide support
- Recognize and report changes in patient condition to the RN or physician promptly
- Maintain compliance with all policies, procedures, and regulatory requirements
- Assist with obtaining and tracking authorizations for services, working with the clinical and administrative teams
- Current LPN license in the state of VA
- Minimum of one (1) year of LPN experience; home health experience preferred
- Valid driver’s license, reliable transportation, and proof of auto insurance
- CPR certification (BLS) required
- Excellent communication, clinical, and organizational skills
- Ability to work independently and manage time effectively
