SUMMARY

This position provides diabetes education and diabetes management for patients. Participate as a member of the health care team and as directed by referring physicians to meet patient needs and provide continuity of care.

To perform this job successfully, an individual must be able to perform each responsibility satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability to be successful.

DUTIES/RESPONSIBILITIES (include but are not limited to the following)

  • Provide direct care, education, and counseling for inpatients and outpatients as directed by patients physicians.
  • Participate as a member of the multi-disciplinary team to develop, implement, and evaluate individual care plans.
  • Document care and activities in the patients medical record in a clear, complete, concise, and timely manner.
  • Serve as a patient advocate to resolve concerns or complaints.
  • Responds to appropriate medical emergencies.
  • Assist in the development, and implementation of diabetes-related programs and materials for patients, staff, community, and other professionals.


EDUCATION AND/OR EXPERIENCE

  • Current RN licensure to practice in the State of Iowa is necessary.
  • Achievement of Certificate as a Certified Diabetes Care and Education Specialist (CDCES), preferred, but not required
  • Minimum of five years of nursing experience.

SKILLS AND ABILITIES

  • Excellent written and verbal communications skills
  • Ability to work both independently and as part of a team, and to make independent judgments/decisions regarding patient care, within own scope of practice
  • Knowledge of diabetes management and education principles
  • Builds a logical approach to address problems or opportunities by drawing on ones knowledge and experienced based, and calling on other references and resources as necessary.
  • Changes behavioral style or method of approach when necessary to achieve the best interest of the patient.
