Position Summary

WFS is currently looking for a Foster Care Coordinator for our Foster Care & Adoption Program. Working under the clinical direction of the Program Supervisor, the Foster Care Coordinator performs a wide range of supportive services within Allegheny County for children in foster homes and treatment foster homes. Provides support and interventions to the child and birth/foster families. Participates and represents the agency, child, child’s birth family, and foster family at permanency planning and team meetings. Develops Individual Service Plans and facilitates achievement of the identified objectives.

This position is a Primary Driver position and as such the incumbent is subject to yearly Motor Vehicle Review Screenings as per WFS Policy on Vehicle, Driving, and Equipment – Safety and Usage.

Education

· Bachelor Degree in Social Work, Child Development, Counseling, or related field

· Significant experience working with at risk children and/or within the Foster Care/child welfare system will be considered with any degree.

Experience

· Minimum of 1 year in a treatment setting delivering services to special needs children and families

· “Satisfactory” driving record as defined by HR Policy #410 - Vehicle, Driving, and Equipment – Safety and Usage

  • Completes assessments on all children entering foster care to include social assessment, ISP assessment, and development assessment.
  • Monitors a caseload of children placed in foster/IRT homes.
  • Assesses the child’s needs and safety. Develops and implements an Individual Service Plan. Reviews and revises the plan periodically.
  • Oversees service delivery to children with special needs, medically needy children, children with mental health diagnosis, and/or children with developmental delays.
  • Interacts regularly with children in placement, as defined by their assigned placement Level of care, and assists each individual in meeting the goals of the Individual Service Plan.
  • Arranges for the required internal and external linkages to support the child in the foster/IRT home settings and coordinates the delivery of other needed services, such as special education to facilitate achievement of goals. Arranges ISP/Treatment team meetings with all parties
  • Supports the foster/IRT family in its effort to meet the child’s developmental and personal needs through regular weekly contact with the foster family.
  • Coordinates and supervises visitation between children and their families of origin.
  • Provides counseling services and coordinates the delivery of other needed services, such as special education as defined by the FSP, CPP, ISP, treatment plan, and court order.
