What are the responsibilities and job description for the Hospice Chaplain - Per Diem position at West Coast Hospice & Palliative Care?
As our growth continues, West Coast Hospice is seeking a Chaplain in Carlsbad and surrounding areas, to aid in our mission: to provide cost-efficient, quality home care services to the patients entrusted in our care. As a Per Diem Hospice Chaplain you will:
- Assure spiritual assessment of patients and families/caregivers in the hospice program; identify spiritual needs
- Provide direct spiritual care to patients and families/caregivers
- Submit concise and timely documentation per agency standards
- Attend interdisciplinary group meetings as a member of the team; provide consultation, education, and support to the team regarding spiritual care
- Attend funerals and memorial services for patients as requested
- Provides bereavement support as needed
- Follow company policies, procedures and directives
- Interact in a positive and constructive manner
- Prioritize and multitask
Hospice is both challenging and rewarding! This job requires a person who will never forget that home care is a team effort. Other requirements include:
- Graduate of accredited seminary or school of theology, Certified Chaplain, or other experience in spiritual counseling
- Hospice experience preferred
- Ability to communicate effectively, both verbally and in writing
- Respecting the patient’s privacy and maintaining confidentiality
- Current California driver’s license and automobile liability insurance
