Summary: This position is responsible for the coordination, monitoring, and facilitation of services designed to reduce suicide risk and improve mental health outcomes among high-risk individuals. As part of the Zero Suicide team, the Case Coordinator ensures that clients receive timely, integrated, and continuous care, working closely with mental health professionals and peer support staff to address individual needs. The role involves conducting risk assessments, developing individualized care plans, and tracking client progress in alignment with the Zero Suicide framework.
Essential Duties and Responsibilities include the following. Other duties may be assigned to meet business needs.
- Serve as the primary contact for clients identified as high-risk for suicide, providing person- and family-centered care. Conduct comprehensive risk assessments and ensure that each client has an Individual Treatment Plan (ITP) focused on suicide prevention.
- Coordinate and lead interdisciplinary team meetings to ensure the development, review, and revision of treatment plans in line with client needs. Facilitate communication between clients, family members, mental health professionals, and peer support specialists to ensure seamless care transitions.
- Assist in developing and implementing personal crisis safety plans with clients. Provide crisis assistance as needed, including conducting suicide risk assessments, making appropriate referrals, and ensuring that clients are connected with emergency services during times of crisis.
- Advocate for and assist clients in accessing necessary mental health, physical health, and social services. Coordinate referrals to behavioral health services, suicide prevention programs, and other resources within the Zero Suicide framework.
- Regularly monitor and follow up with clients to ensure they are receiving the appropriate level of care. Track progress against the treatment plan and provide supportive counseling as needed, ensuring continuous engagement in care and early intervention when signs of suicide risk emerge.
- Accurately document all services provided, including assessments, care plans, referrals, and follow-ups, in the electronic health record (EHR). Ensure compliance with Zero Suicide data collection and reporting requirements, contributing to system-wide evaluations and quality improvement.
- Provide education and training to clients, families, and staff on suicide prevention strategies and the Zero Suicide framework. Participate in community outreach to raise awareness of suicide prevention resources available to clients.
- Assist clients with transportation needs, including arranging transport to appointments or personally transporting clients when necessary to ensure they access care promptly.
- Adhere to federal, state, and organizational policies, procedures, and regulations related to client care. Participate in Continuous Quality Assurance and Utilization Management efforts, ensuring that services meet the highest standards of care.
