What are the responsibilities and job description for the Direct Support Professional position at Westbrook Health Services, Inc?
Summary: Position responsible for direct, basic, and supportive care to Individuals with Intellectual/Developmental Disabilities (I/DD) within assigned program.
Essential Duties and Responsibilities:
- Provides training, health monitoring, assistance with activities of daily living (ADLs) and transportation for I/DD Individuals.
- Provides 1:1 – 1:3 oversight and intervention to Individuals experiencing major behavioral health or other crises.
- Serves as a member of the support services team attending all meetings.
- Maintains daily logs and records of patient care, condition, progress and/or problems to report, and discusses observations with Supervisor or Program Director.
- Monitors Individual safety needs, including physical, medical, behavioral, and psychiatric.
- Monitors Individuals to ensure their safety by observing health and vital signs, including blood pressure, pulse, temperature, and respiration. Documents Individual behaviors according to the Individual Program Plan (IPP)
- Maintains facility to ensure sanitary conditions.
- Assists with cooking, laundry, and other general cleaning activities necessary to maintain sanitary conditions and general upkeep.
- Accompany Individual to doctor’s offices and on other trips outside the facility providing transportation, assistance, and companionship.
- Comply with federal, state, and company policies, procedures, and regulations.
- Maintains confidentiality of all information and complies with HIPAA regulations.
- Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards, and code of ethical behavior.
- Other duties may be assigned to meet business requirements.
