Licensed Practical Nurse (LPN)

0.75 - 1.0 FTE, 60 - 80 Hours a Pay Period
Days, Hours between 5 am - 5 pm
Weekends: Every Other Weekend (Friday-Saturday)
Holidays: Every Third Holiday
Position Overview:
The Licensed Practical Nurse (LPN) is a health care professional who provides specified services in the Dialysis Department under the supervision of a Registered Nurse (RN). The LPN is active in the nursing process and collaborates with the RN to deliver quality and safe care. The LPN understands the Nursing Practice Process including assessment by the Provider & RN, planning, implementation, and evaluation of the Dialysis patient. The Dialysis LPN is also active in the technical process of dialysis under the direction of the RN.
Essential Duties & Responsibilities:
- Performs technical aspects of dialysis care delivery:
- Sets up and operates hemodialysis machines to provide dialysis treatments;
- Attaches dialyzer and tubing to the machine to assemble for patient use;
- Performs cannulation of dialysis vascular access or connection to dialysis catheters and initiates hemodialysis treatment;
- Transports patients as needed;
- Takes and records patient pre and post treatment weights, vitals, and other pertinent health information;
- Completes necessary regulatory testing of equipment and records as needed;
- Calculates fluid removal or replacement to be achieved during treatment
- Collaborates with the RN to modify the plan of care based on the needs of the patient and reports changes in the patient’s condition to the RN.
- Keeps updated on current Dialysis nursing practice, medications, and legal issues related to patient-based services.
- Conforms to the Nurse Practice Act.
- Follows recommended infection control procedures during patient contact.
- Is constantly vigilant to the safety needs of the patient, taking necessary precautions as warranted.
- Provides safe, cost effective, and holistic care to patients and recognizes abnormal conditions and initiates appropriate interventions.
- Documents all pertinent data in the EMR. Reports malfunctioning equipment promptly and removes from patient care area to maintain a safe environment.
- Participates in and supports continuous improvement initiatives.
- Acts as a resource to students.
- Answers patient call lights and addresses patient needs.
- Other job duties as assigned.

Skills and Experience:
Required:
- Associates Degree in Nursing
- LPN License in Minnesota
- BLS Certification
- Verbalizes role in public address codes
- Completes and passes the “Ishihara Test for Color Deficiency” prior to employment
- Basic computer skills: Microsoft Word and Outlook. Patient Information Database (Oracle/Cerner)
