Job Title: Housing Specialist - Wellness & Empowerment
Job Overview :
The Client Engagement Services (CSH) program provides outreach and engagement activities to adult living with HIV/AIDS who are unhoused. The program targets those in the shelter system in need of securing stable housing. The program provides linkage to medical care and other support services such as housing placement assistances, mental health services, entitlement and benefits, food and nutrition services and other needs as identified by participant.
Primary Duties & Responsibilities include, but are not limited too:
- Recruit, screen and enroll new participants in the CSH program
- Conduct intake/enrollment and ensure documentation of eligibility criteria is met
- Develop charts for each new participant and document all encounters
- Prepare weekly and monthly reports
- Send daily census report to supervisor
- Track enrollment and activities
- Coordinate with HRA/HASA worker to ensure participant housing packet is ready
- Assist participants with completing housing applications
- Escort participant to view apartments
- Remind participants about appointments
- Facilitate bi-weekly housing group to discuss housing placement and retention
- Other duties as assigned by supervisor
- Familiar with housing resources and placement such as 2010e, transitional housing options etc.
- Knowledge of harm reduction services
- Excellent communication skills
Education:
- Associate degree with experience working in community setting for at least a year
Salary : $27 - $28
