As a Case Manger, you will provide and document restorative services to participants of the OMH licensed Apartment Treatment Program on site and off site, to assist in their transition toward independent living by reinforcing and shaping learned skills.

What your day might look like:
- Provides accurate recordkeeping and client advocacy which includes coordination of services within agency, i.e. funding issues, medication programming, daily living skills training, progress notes, goal planning and providing requirements for Medicaid billing
- Maintains medication records for individual client’s on a weekly basis and monitor all incoming medications by completion of appropriate logs and daily controlled substances counts
- Observes possible malfunctions of equipment and contents of apartment; takes appropriate action when needed
- Acts as a liaison between client and landlord to resolve maintenance issues quickly
- Assists residents in maintaining apartments in a safe and sanitary condition
- Conduct a fire drill on every resident once a month and check smoke alarms on a weekly basis
- Complete inspections of units to ensure units are safe and complete documentation required
- Manages a resident caseload to include all areas of case management, including effectively coordinating/communicating needs to treatment team members including funding agencies, providers, etc.
- Makes daily/weekly visits to apartments on an as needed basis
- Provides ongoing support and referrals to community services as needed
- Responds immediately to crisis situations and offer intervention support in conjunction with BHSN Crisis and/or treatment team providers
- Adjusts schedule to meet the needs of the program and ensure support services are provided
- Transports residents to and from doctor appointments, grocery shopping, etc., while teaching residents on how to acquire independent transportation when possible
- Complies with all BHSN and regulatory policies and procedures
- Other duties as assigned
Your skills and qualifications:
- Experience with mental health or related field required
- Strong interpersonal, organizational and time management skills
- Valid NYS driver’s license
- Reliable transportation to accommodate program and/or resident needs
- High School Diploma or equivalent
- Ability to climb stairs
- Participate in lifting and moving a resident’s belongings
- Required to complete 5 hours annually toward continuing professional/educational development
- Includes work hours during non-traditional business hours

Salary : $32,700 - $41,500
