What are the responsibilities and job description for the Care Coordinator position at CDAC Behavioral Healthcare?
CDAC Behavioral Healthcare is expanding its Substance Exposed Newborn/Neonatal Abstinence Syndrome (SEN/NAS) program into Circuit 02 (Gadsden, Liberty, Franklin, Leon, Wakulla, and Jefferson - we will not serve Madison or Taylor at this time). This program supports families who are experiencing, or at risk for, SEN/NAS.
CDAC is seeking two (2) Care Coordinators to provide care coordination services to women and families with substance use concerns in Circuit 02 counties defined above. The position requires clinical skills and experience with substance use and mental health disorders, parenting and family issues, and community resources. Candidate should have a working knowledge on the impact to families with substance exposed newborns (SEN) and Neonatal Abstinence Syndrome (NAS). As part of the SEN Team, the Care Coordinators will coordinate and assist in addressing all the needs of the families to include substance use, mental health, medical, financial stability, and overall wellness. The Care Coordinator will work closely with community providers and with child welfare providers to advocate and assist in meeting these families' needs. The candidate must demonstrate an understanding of ROSC (recovery-oriented systems of care) principles, case management and care coordination principles, and have experience working with people with substance use and mental health concerns.
Two (2) positions available serving the defined counties (Gadsden, Liberty, Franklin, Leon, Wakulla, and Jefferson) of Circuit 02.
Bachelor's degree in social services field with at least two (2) years of relevant experience required. Master's degree preferred.
All work must be completed within the state of Florida.
