FACT Vocational Specialist

Description

• Provides services to help individuals with mental health/substance use disorders to thrive in the community and focus on recovery.
• Provides the rehabilitation and support functions under clinical supervision.
• Integral member of the treatment team.
• Provides social and communication skills training and training to enhance patient’s independent living.
• Provides substance use, educational, and support services to patient and family members, as approved by patient.
• Crisis intervention, as required.
• On-going assessments, problem solving, assistance with activities of daily living (ADL), and coaching.
• Provides case management services and individualized treatment planning
• Serves in on-call rotation for FACT team to provide after-hours crisis support and intervention to patients.
• Assess all individuals for eligibility for SSI, SSDI, VA benefits, housing benefits, and public benefits and assist them in obtaining eligible benefits; When applying for SSI and SSDI benefits, uses the SOAR application process.
• Provides Vocational Rehab Services for FACT patients
• Provides coaching to other staff regarding the SOAR process
• Provides assessment of needs.
• Develops service plan based on identified needs and monitors implementation.
• Links individuals to appropriate community service agencies.
• Responsible for ensuring individuals are receiving appropriate services, based on identified needs.
• Coordinates effective transition/discharge planning.
• Coordinates with caregivers in reference to individual needs, as appropriate.
• Provides services in individual/group homes and other community locations.
• Documents all interactions with and on behalf of the individual served in the electronic medical record (EMR) within 48 business hours of providing services.
• Transports individuals served, when needed, in agency or personal vehicle.

Qualifications

• Bachelor’s degree in a behavioral science
• One (1) year experience with adults with psychiatric diagnoses.
• CBCM certification preferred.
• Ability to pass a level II Background clearance and drug test.
• Valid FL drivers’ license, insurance, and safe driving record.
• CBHC New Hire Orientation (NHO) completed within first two (2) months of employment.
• Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures.
• Ability to use a computer.

Salary : $19 - $23
