Description
- Provides practical advice, suggestions, and strategies of recovery to patients as an empowering helper with the perspective of being a recipient of services in the past.
- Provide one-on-one support.
- Work with individuals and families to prevent crisis and promote treatment engagement, while assessing risk ongoing.
- May help individuals deal with addictions and substance abuse; family, parenting, and marital problems; suicide; stress management; financial issues; and issues associated with the aging and mental and emotional health. Must be very familiar with and actively refer to various social services resources in Charlotte and surrounding counties
- Provide hope, support, education, and assist patients in setting recovery goals.
- Employee will work closely with law enforcement, Department of Children and Families and other local, state and federal service providers to coordinate services for individuals determined to be in need.
- Employee will make appropriate referrals, monitor the implementation of services, and provide support and advocacy for the individual.
- Must be very familiar with and actively refer to various social services resources in Charlotte and surrounding counties.
- Provide input, as part of the treatment team, on the patient’s care.
Qualifications
- High School Diploma or equivalent.
- Peer Specialist certification, preferred. Certification is required within one (1) year of hire date.
- Ability to lift at least 10 pounds.
- Ability to reach above shoulder level.
- Moderate manual dexterity.
- Ability move quickly during a crisis.
- Ability to sit and stand for long periods.
- Ability to safely operate a motor vehicle in all conditions.
- Ability to pass Level II Background clearance, drug test (CBHC is a drug free workplace)
- Valid FL driver’s license, insurance, and safe driving record
- Able to manage stressful situations and display appropriate work demeanor and boundaries
- Must demonstrate exceptional organizational skills and abilities that enable a case manager to work creatively and independently while coordinating multiple, complex, priority crisis situations
- Demonstrate excellent customer service
- Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures
- Must have valid Florida driver’s license that reflects a safe driving record and use of personal vehicle
- Must have good computer skills in order to enter and retrieve information into an electronic medical record
- Strong oral and written communication skills.
- Ability to work both independently and as part of a team
