Job Description



POSITION TITLE: Residential Programs Ryan White Medical Case Manager

CLASSIFICATION STATUS: Exempt


POSITION DESCRIPTION: Manage an assigned caseload of clients living with HIV/AIDS requiring comprehensive intensive case management, Ryan White Medical Case Management Services, and care coordination for clients living in the Supportive Living Program, the Family Support Program, and the Independent Living Program.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Manage an assigned caseload of 25 to 45 clients living with HIV/AIDS requiring comprehensive, intensive case management, Ryan White Medical Case Management Services, and care coordination for the clients living in the Supportive Living Program, the Family Support Program, and the Independent Living Program.
  • Coordinate care between other service providers including, but not limited to: doctors, hospitals, insurance companies, and pharmacies.
  • Collaborate with program staff to assure quality care and to identify appropriate services for the clients.
  • Maintain a professional therapeutic relationship with the clients and practice trauma-informed care and strengths-based approaches.
  • Collaborate with clients to establish Individualized Service Plans using SMART goals every six months and review progress on their goals with them every 2 weeks.
  • Conduct intakes, psychosocial assessments, and self-sufficiency assessments within 7 days of the client being housed in the program.
  • Obtain Medical, Viral Load and CD4 testing results from medical providers every six months and document if attempts are not successful.
  • Collect information on health insurance, income, residency, and adherence to medical appointments, medications adherence, behavioral health and substance abuse, oral health, nutrition, transportation, and basic needs on an ongoing basis from the clients.
  • Provide and document referrals and link clients to other appropriate resources.
  • Create and maintain resource and referral networks.
  • Follow up with client's service providers regarding referrals.
  • Coordinate service delivery in a timely manner.

  • Respond to issues effectively and timely.
  • Notify Chicago House Supervisor of complaints from clients.
  • Notify Chicago House supervisor ASAP if any clients express suicidal ideation or intention and/or any plans to harm themselves or others.
  • Document activities regularly in progress notes within 5 days.
  • Enter Ryan White/ADAP eligibility assessments in Provide, the client database, every 12 months. Record activity as required in Provide (progress notes, care plans and acuity scales every 6 months, wellness questionnaires, medical and other health appointments, referrals, services provided, transportation provided, test results, bill payment requests, change in client information including address, contact information, income, and also medications, and other activity as needed).
  • Protect the oral, written and electronic confidentiality of the client and follow HIPAA guidelines.
  • Participate in case conferences to sustain or improve client quality of life with the client's health care and mental health providers.
  • Participate in blended management meetings between property management and program staff, as needed.
  • Participate in IDT Inter-Disciplinary Team) meetings with residential program staff.
  • Provide referrals to other AIDS Service Organizations and resource providers.
  • Maintain client files in accordance with the standards established by Chicago House and Social Service Agency, AFC, IDHS, HOPWA, COA, and additional funders.
  • Attend and participate in staff meetings and program events.
  • Attend Case Management meetings at Chicago House and the AIDS Foundation of Chicago, in person and/or virtually, as required and establish networking linkages with attending case managers
  • Report health and/or behavior changes of clients to the Residential Program Managers, Director of Residential Housing, and Manager RW Medical Case Management (as appropriate).
  • Complete trainings as required by the AIDS Foundation of Chicago and Chicago House.
  • Participate and help organize community meetings for clients at the residential buildings.
  • Attend events at residential buildings, such as monthly community meetings, game nights, family engagement nights, holiday events, cookouts, etc. as able.
  • Other duties as assigned.



REQUIREMENTS/QUALIFICATIONS:

  • Bachelor's degree in Social Work or a Bachelor's degree in a human service field with a minimum of 2 years case management experience. Master's Degree in Social Work is preferred.
  • Commitment to Trauma-Informed Care and Strengths-Based Approaches.
  • Familiarity with HIV/AIDs, Mental Health, Substance Use, and affirmative practice with LGBTQ individuals.
  • Ability to work cooperatively as a part of a team in a clinical and non-clinical setting.
  • Ability to work a flexible schedule at times.
  • Excellent oral and written communication skills.
  • Excellent organizational and time management skills.
  • Excellent computer skills, including how to use effectively and easily Microsoft Windows, Outlook, Word, and Excel.
  • Teamwork-oriented.
