What are the responsibilities and job description for the Adult Case Manager position at Clara Martin Center?
Seeking a fun, energetic, full time human services professional to provide case management, outreach and referral services to adults with mental health and/or substance abuse related issues. Services are provided on an outpatient basis in the office and in the community. Case Managers are part of a team of professionals including Therapists, nursing, vocational and support workers.
Essential functions include: Providing case management, care coordination, supportive counseling & problem solving to adults, groups and family members.
- Documenting clinical services in an electronic medical record.
- Gathering & completing paperwork to open charts, document progress and discharge when appropriate.
- Providing outreach & support to clients in the community.
Job Type: Full-time
Pay: From $21.00 per hour
