FIT Case Manager
Provide initial and ongoing guidance, systems education, and support to clients to encourage treatment engagement and retention. Current status (verbal or written) reports will allow for either FIT to assist in reengaging parents and giving parents credit for their efforts towards their recovery and reunification plans.
Job Tasks & Duties:
- Serves as liaison between clinical staff and ODHS caseworkers.
- Communication can be supported between child welfare and treatment providers.
- Supports clients in identifying concrete needs during treatment and addressing them. Reaches out FIT liaisons with ODHS staffing concerns and resource requests as needed.
- Example: Parent Mentor referrals, transportation needs, bridging communication with child welfare
- Submits CORE Team Communication (CTC) form to ODHS FIT staff with all information and dates regarding assessment, intake, first treatment status, entry into residential (if applicable), and any change in recommended treatment level or engagement status and discharge/completion dates.
- Reports & notifies ODHS caseworker and FIT team members within 48 hours when there is a loss of contact with a client, the client has missed 2 treatment groups, client’s failed to attend treatment sessions, or they leave residential programs. Communicate these findings by phone, email, and CTC form communication.
- Collaborates with FIT Recovery Navigators to designate and direct the clarification of roles and responsibilities with specific clients, such as who performs what task and who is communicating that information to clients.
- Completes & submits Community Mentor referral requests for clients who may need or want additional help during or after treatment and build community recovery connections.
- Completes & submits Monthly Treatment Progress Reports by electronic mail to the DHS FIT team to add as case documents. These need to be received by the 10th of the following month.
- Attends clinical staffing at treatment centers for supervision.
- Attends ODHS /FIT Case Manager staffing meetings
- If this needs to be rescheduled, please contact FIT liaison ASAP
- For a most productive meeting, be prepared with active client list to review current treatment updates from treatment counselors. This information should be documented also on CTC and/or Monthly Reports.
- Attends client court hearings to provide clinical updates and to support to client.
- Attends monthly FIT Case Manager Team meetings with FIT staff, as well as the monthly Operations Committee meetings (3rd Thursday of the month).
- Identifies the need for child visits and problem solves scenarios to make visits more successful.
- Refer to FIT Liaison Connection email for names and contact information for ODHS case worker, Recovery Navigator, visitation information and other details.
Qualifications
REQUIRED EDUCATION AND EXPERIENCE:
- Bachelor’s Degree in Social Work, Psychology, Criminal Justice or closely related area of study

- Adhere to regulatory requirements and maintain professional, ethical and behavioral standards of conduct.
- Ability to be attentive to and identify high risk situations, communicating with the appropriate channels in a timely manner.
- Ability to intervene in such a way that demonstrates sensitivity and respect while keeping a clear line of communication between supervisor and treatment team.
- Ability to demonstrate a non-judgmental and respectful attitude with patients at all times.
- Ability to establish rapport quickly, maintain appropriate boundaries, and remain nonjudgmental to patients.
- Ability to utilize both oral and written communications skills to effectively communicate with patients, co-workers, community resources, supervisors, and various other individuals encountered in the course of performing work while maintaining patient confidentiality and establishing professional rapport.
- Ability to maintain professional communications and boundaries while managing complex situations and interacting with individuals diagnosed with substance use or co-occurring conditions.
- Ability to operate a computer, utilize basic MS Office programs, and effectively navigate and maintain patient records in an electronic medical records system.
Salary : $49,233 - $54,200
