Do you have the desire to provide a wide variety of services to the elderly population and want to make a difference in their lives? If so, Luzerne County Area Agency on Aging is looking to hire a compassionate, patient, and professional individual to join our team!


This Aging Care Manager 2 will conduct field visits in order to respond to emergency situations, monitor services as specified in the care plan, provide emotional support to consumers, resolve problems, etc. The employee will also need to arrange for financial management for the consumers who are unable to handle their own finances. This may include setting up budgets with consumer participation or arranging for an agency to act as payee for a consumer's public benefits. Other duties include but are not limited to assisting consumers in negotiating the Aging service system or assisting consumers with pre-placement and placement activities when community services are no longer sufficient to maintain consumers in the community.

Work Schedule and Additional Information:
  • Full-time employment.
  • Work hours are 8:00 am to 4:00 pm, Monday - Friday, with a 30-minute lunch.
  • Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS
Minimum Experience and Training Requirements:
  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy will be given to candidates who live within Carbon, Columbia, Lackawanna, Luzerne, Monroe, Schuylkill, Sullivan & Wyoming counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
