What are the responsibilities and job description for the Care Coordinator - Mental Health position at CRF Behavioral Healthcare?
Hours: 40 hours per week. Hours are Monday, Tuesday, Thursday, and Friday, 8:30 am to 5:00 pm; and Wednesday, 10:30 am to 7:00 pm.
Pay Rate: $28.85 to $29.13 per hour
Education: Bachelor’s degree in a mental health-related field such as Psychology, Social Work, or Counseling required

As a Care Coordinator, you will:
- Work closely with various providers in the region or system to facilitate access to program services for those identified as the target population to be served.
- Conduct initial assessment of case management needs for all new clients and/or returning clients.
- Provide care coordination for clients and/or members of clients' families to support the client.
- Inform program staff of any concerning issues regarding crises and/or unusual incidents.
- Develop and maintain positive and professional working relationships with community health, welfare, and social agencies.
Additional Qualifications and Skills
- At least one year of related experience and/or training
- Must be able to work in a fast-paced and collaborative environment
- Bilingual Spanish preferred, but not required
- A valid California driver’s license in good standing, personal vehicle, proof of insurance and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position
- Proficient computer skills, including MS Office
- Able to pass criminal/background clearance

Our Adult Outpatient Programs (Areta Crowell Wellness Recovery Center, Douglas Young Wellness Recovery Center, Heartland Wellness Recovery Center, Jane Westin Center, Maria Sardinas Wellness Recovery Center, and South Bay Guidance Center) provide behavioral health services including psychiatric and clinical assessment, treatment planning, ongoing medication services, individual and group counseling, case management, crisis intervention, mobile outreach, vocational support, and peer support. We deliver fully integrated services to adult clients diagnosed with a serious mental illness, as well as individuals with co-occurring, mental health, and substance use disorders.
All of our outpatient programs also provide full-time walk-in services. Our programs utilize evidence-based practices to assist our clients in reaching their goals and to live the highest quality of life while managing their mental illness.
