 Pathways to Success is a program for Medicaid-enrolled youth in Illinois who have complex behavioral health needs requiring intensive services.  Pathways to Success is an evidenced-informed model of intensive care coordination to support youth through family engagement; individualized care planning; and the utilization of strengths and natural supports. DuPage County Health Department serves as a Care Coordination and Support Organization and operates as a hub for care coordination for Pathways-enrolled individuals within DuPage County.  

 An individual in the Care Coordinator position will be responsible for providing care coordination and support services to clients enrolled in the Pathways to Success program.  The position involves working collaboratively with youth, their families, and community providers to achieve client goals.  This full-time position works 37.5 hours per week Monday through Friday, some evenings may be required to meet training and coaching needs of staff.

  • Provides care coordination and support services to Pathways-enrolled youth and their families
  • Manages a caseload of clients as defined by the Pathways to Success Program
  • Maintains weekly contact with clients/families
  • Develops child and family teams for clients enrolled in the program and facilitates regular team meetings 
  • Provides homebased and community-based services to engage at risk clients and support client self-management of their illness 
  • Develops and maintains client treatment plans 
  • Participates in all required HFS planning, training, and reporting activities.   
  • Works collaboratively with community providers
  • Use of an Electronic Client Record 
  • Demonstrates independent judgment and discretion in making position related decisions
  • Participates in emergency response activities as assigned
Completion of a bachelor’s degree in Psychology, Social Work or a related field and one year of behavioral health experience; or an equivalent combination of training and experience.
A valid Illinois Driver's License is required as this position will work within the community. Spanish speaking candidate highly preferred.

Salary : $1,154,400 - $1,414,400
