Job Summary:
As a Physical Therapist in our homecare team, you will provide high-quality physical therapy services to patients in their homes under the guidance of qualified physicians. You will assess patient needs, develop and implement personalized care plans, and evaluate progress towards therapeutic goals.
Duties and Responsibilities:
- Conduct thorough assessments of patients' physical abilities and develop appropriate care plans
- Provide one-on-one physical therapy sessions to patients in their homes
- Evaluate patient progress and adjust care plans as needed
- Collaborate with healthcare teams to ensure comprehensive patient care
- Maintain accurate and timely documentation of patient interactions and progress
Requirements:
- Master's or Doctoral degree in Physical Therapy from an accredited program
- Current license to practice as a Physical Therapist in Connecticut
- One year of acute care experience; home health care experience preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Salary : $85,000 - $105,000
