- Perform home visits to assess the patient’s health needs and implement a nursing care plan.
- Responsible for delivering a quality medical care in compliance with industry/company regulations, policies, and procedures.
- Administering medications to patients and monitoring patients for reactions or side effects.
- Monitoring all aspects of patient care, including diet and physical activity.
- Modifying patient treatment plans as indicated by patients' responses and conditions.
- Provide education to patients and families on proper home health care procedures and strategies.

- Proven record of providing patients with nursing care.
- Ability to exercise independent and sound judgement.
