The ideal candidate will work collaboratively with the interdisciplinary team to ensure accurate assessments, high-quality care planning, and regulatory compliance.
Key Responsibilities
- Conduct, coordinate, and complete all resident assessments (MDS) in accordance with current regulations, including RAPs and Triggers.
- Ensure all MDS assessments are completed, transmitted, and maintained in a timely and accurate manner.
- Maintain and update written policies and procedures governing the MDS and care planning process.
- Collaborate with the Interdisciplinary Care Plan Team to develop comprehensive, individualized care plans.
- Assist with discharge planning and completion of care plan components related to resident discharges.
- Monitor Quality Indicator (QI) and Quality Measure (QM) reports and initiate corrective actions when necessary.
- Develop, implement, and maintain an ongoing Quality Assurance program related to assessments and care plans.
- Participate in facility surveys and inspections conducted by regulatory agencies.
- Serve on and participate in facility committees such as Quality Assessment and Assurance and Policy Advisory, as assigned.
- Provide written and verbal reports regarding resident assessments and care plans as requested.
- Coordinate scheduling and participation of internal and external care plan team members, including residents and family representatives.
- Assist in evaluating residents’ daily living skills, abilities, and limitations.
- Work closely with the Director of Nursing Services to ensure timely notification of newly admitted residents.
- Maintain professional, cooperative relationships with physicians, consultants, health professionals, and regulatory agencies.
- Active, unencumbered Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Florida.
- Minimum 2 years of supervisory experience in a hospital, skilled nursing facility, or related healthcare setting.
- Minimum 6 months of training or experience in rehabilitative and restorative nursing practices.
- Prior nursing home experience required.
- Strong knowledge of nursing practices, medical procedures, and applicable laws and regulations.
- Ability to make independent decisions and manage multiple priorities.
- Proficient in PointClickCare.
- Ability to read, write, speak, and understand English fluently.
