What you will do at Harris County Precinct 2
Under the direction of the Social Worker Coordinator, this position will assist with the day-to-day operation of the Access2Rides transportation voucher program as well as with health services programming and outreach. Responsibilities include, but are not limited to case management, grant compliance, monthly reporting, maintaining a partnership with RIDES, community outreach and engagement.
Duties & Responsibilities:
Assist with the Transportation Voucher Program, maintaining appropriate files and required reporting for grant compliance.
Assist with completing assigned tasks within Health Services division to successfully maintain programs and activities that improve Health outcomes of Precinct 2.
Assist with Case Management, resource, and referrals for constituents and maintain appropriate files and records for tracking, statistics and grant compliance
Maintain current knowledge of health programs throughout Harris County and City of Houston including current rules and regulations.

Education:
High School Diploma from an accredited college or univerisity
Experience:
Experience working with low income, minority or vulnerable populations
Experience with record keeping and compliance
Experience in customer service
Experience with community programs, social services and or referrals
Knowledge, Skills, and Abilities
Must have strong written and verbal communications skills.
Proficient working knowledge of computers and related software comprised of CIVICREC, Microsoft Excel, Word, Outlook, and office equipment.
Ability to create a safe, secure and healthy work environment for fellow employees.

Ability to demonstrate a high level of professionalism, with excellent communication skills.

Position Type and Typical Hours of Work:
A temporary full-time position, requiring the employee to work about 40 hours per week. This may include weekends and holidays.
- This job mainly operates in a clerical, office setting, however, will require field work for asset capture/identification. This role routinely uses standard office equipment such as computers, phones, photocopiers and filing cabinets. Noise level in the work environment is usually average.
- This position requires sitting, standing, walking, grabbing, reaching, kneeling, bending, stooping, squatting, grasping, and repetitive motions in the performance of daily duties. Requires the ability to lift files of up to 50 lbs, as necessary.
