Location: Berkley, CA and Oakland, CA
- Conduct outreach to eligible patients via phone and in-person at various sites, building relationships and assessing needs.
- Engage patients and caregivers in care plan development, aligning services with patient goals.
- Facilitate referrals, assist with applications, and provide health education (e.g., harm reduction, disease prevention).
- Maintain caseload per program standards, using data tools to track outcomes and meet grant deliverables.
- Assist patients with public benefits (Medi-Cal, SSI/SSDI, housing) and access to medical/behavioral health services.
- Collaborate with care teams, external partners, and patient supports, ensuring ongoing communication.
- Provide case management to patients with complex medical or behavioral conditions (e.g., HIV/AIDS, homelessness).
- Participate in team meetings, crisis interventions, and quality improvement initiatives.
- Mentor and coach case managers, interns, and volunteers; provide onboarding and technical support.
- Document services in required systems (EHR, HMIS), ensuring compliance with policies.
- High School diploma or GED.
- 3 years of experience in community health or social work, or 1 year as a Case Manager II/III, with certification as a Certified Alcohol and Drug Counselor.
- Proficient in Microsoft Office, data management, and project coordination.
- Reliable transportation, with valid license and insurance.
- Experience in training, coaching, and meeting program goals.
