Description
Provide support and assist with access to needed resources and services. Will link individuals to those needed resources and supports in the community such as food, clothing, medical care, employment, education, housing and any other services that they may require.
Schedule: Monday – Friday
Case Manager Responsibilities:
- Assist clients with accessing community resources
- Assess client’s strengths and needs: medical, housing, education, vocation and socialization.
- Monitor client’s progress
- Assist with building a network of supports
- Build professional relationships with community agencies.
- Provide crisis assistance.
- Assist client with advocacy efforts with other community services.
- Maintain appropriate contact with client.
- Assure client choice and access to community resources.
- Attend monthly supervision.
- Return phone calls and emails promptly.
- Complete and submit documentation by required deadline.
- Perform other related work as required and assigned.

Qualifications
BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences. One year of experience working with SMI adults or one year of case management experience, after completing graduation, or 6 months if individual possesses master’s degree in approved field.
Valid KY Driver’s license required, insurance and reliable vehicle.
