Description
RESPONSIBILITIES: The QIDP is responsible for the overall coordination and monitoring of the client’s support and services for assigned clients based on DHS regulations and LFI/Community Living Standards and Best Practices.
SPECIFIC DUTIES:
1. Shall attend and lead client Implementation Strategy meetings and CSTs as required and is expected to attend other Manager’s staff and house meetings as scheduled.
2. Shall be responsible for the overall monitoring of client programming including but not limited to goals, choice-making, and progress for each client, problems in these areas should be discussed with the Director of Program Services/QIDP as soon as possible. Client programming shall be based upon the Personal Plan and individual needs.
3. QIDP will ensure all clients assigned will have an Implementation Strategy whose Strategies, as decided by the Personal Plan and team, meet program licensure and accreditation standards. The Strategies are developed, implemented, and reviewed monthly to ensure updates are completed and client progress on objectives is monitored. This information will be maintained in each client's record by the QIDP.
4. Will develop behavioral intervention programming as needed for clients. Will ensure interventions are developed in accordance with established procedures and approved by the HRC/Behavior Management team as appropriate.
5. Will work in cooperation with appropriate Health Service staff in the monitoring of clients’ general health.
6. Will seek assistance from appropriate sources for improvement and/or modification of services in the following areas including but not limited to, instruction, behavioral support, medical support, nutritional needs, and other aspects of client support, as needed.
7. Shall provide training and resource support to the Community Support Specialists’ and Support Staff in areas including but not limited to, training on client strategies and behavior programs.
8. Convenes meetings of the CST when individual client services/supports need to be reviewed including but limited to, medical, behavioral, social, and financial issues.
9. Shall ensure that all client records are maintained per DHS requirements.
10. Will maintain consistent communication with family members and guardians, to report progress and concerns regarding the individual client. This may be accomplished through parent meetings, phone calls and/or written correspondence.
11. Will maintain contact as necessary with other agency staff including, the Community Supports Manager, Ancillary Support, Health Services and Adult Day Services.
12. Completes the QIDP Monthly report and submits the report to the Director of Program Services/QIDP. The QIDP will also submit weekly schedules for review and approval.
13. Shall be available via Cell Phone for program medical and behavioral emergencies Monday at 8:30 am through Friday at 4:30 pm and maintain on-call responsibilities within established policy and rotation for weekends and holidays. When caring for someone else’s phone, will be responsible for all duties assigned to that QIDP or CSM.
14. Shall be flexible in work hours, including but not limited to minimum expectations of evening and weekend work hours.
15. Shall work shifts in home as needed to ensure coverage at the home is adequate.
16. Consult with the Director of Program Services/QIDP regarding proposed changes in programs, policies, schedules, and procedures and implement such changes when approved.
17. Shall attend all in-service training sessions to expand his/her knowledge of the individual with developmental disabilities, current modes of treatment, and special topics. Will maintain Continuing Education Units (CEUs) per state requirements.
18. Shall attend regular staff meetings to address individual clinical issues in the homes. Shall participate in committees/teams as assigned.
19. Shall be responsible for other duties as assigned by the Director of Program Services which are consistent with the job duties and responsibilities
Requirements
- Bachelors or master’s degree in special education, psychology, or related field with at least one year of experience working with people with developmental disabilities
- Experience in a residential setting and prior supervisory experience is preferred, though not essential.
- Must have a valid Illinois driver’s license, dependable transportation, and maintain required automobile insurance as well as a good driving record
