Ray Graham Association is currently looking for a Case Manager/QIDP to provide services for people with Intellectual and Developmental Disabilities. The Case manager will develop and oversee the implementation of supports and services based on the personal goals and dreams of the people supported.
- Flexible and hybrid work schedule
- Most weekends off
- Not a supervisory or on-call position
Primary responsibilities include, but not limited to:
- Being the hub of communication for families, guardians and agencies
- Navigating and fostering relationships to establish services.
- Working with medical professionals to establish the best possible care.
- Performing and evaluating assessments that measure personal outcomes.
- Creating plans and assisting people supported with opportunities to reach their goals and dreams.
Required Qualifications:
Per Illinois Department of Human Services, must have at least one year of experience working directly with people developmental disabilities and a bachelor’s degree in a Human Service field.
Other Requirements:
- Excellent oral and written skills are essential.
- Must have clear State Police history and Health Care Worker Registry background check
- A valid Illinois driver’s license.
- Must pass a pre-employment drug screen.
