Summary
Better Birth Outcomes – Comprehensive (BBO-C) provides pregnant, and parenting clients with direct, comprehensive, 1:1 nursing assessments and assistance to connect and engage with desired services and understand the importance of a medical home. The Nurse Navigator provides a comprehensive nursing assessment and any immediate nursing intervention necessary. Navigators provide follow-up to dyads (pregnant and postpartum individuals and their infants), helping to connect families to needed supports available in the community. Navigators help the client meet their family’s health and wellness goals and promote the reduction of maternal and infant morbidity and mortality.
Responsibilities
- The Nurse Navigator provides a comprehensive nursing assessment and any immediate nursing intervention necessary.

Requirements
- Minimum of Bachelor Degree in Early Childhood, Social Work, Health Studies, Psychology, or related field.
- Experience preferred in child, family, or community services.
