Responsibilities

• Provide nursing care and health education to individuals and families within the community.
• Manage cases effectively, coordinating care for patients with diverse health needs.
• Utilize electronic health records to document patient interactions and maintain accurate records.
• Collaborate with other healthcare professionals to develop comprehensive care plans.
• Conduct outreach programs aimed at promoting healthy practices and disease prevention.
• Assist in the management of public health initiatives and emergency response efforts.

Qualifications

• Registered Nurse (RN) license in Illinois or eligibility for licensure.
