Job Description Summary: As a homecare RN at MaineGeneral, you will focus on promoting patient well-being, independence, and quality of life in the patient's home setting. You will also play a crucial role in supporting patients with chronic illnesses and providing end-of-life care when needed. Job Description: $15,000 sign-on bonus potential for experienced RNs Must have one year of RN experience Must accept at least a 30 hour-per-week position. Current MaineGeneral employees are ineligible; former MaineGeneral employees are ineligible until greater than 6 months of separation from employment. We’re currently seeking a Registered Nurse (RN) for a Hospice Case Manager position on our Hospice South Team based out of Augusta. This position is available for 32 to 40 hours per week (candidate choice) with a weekday schedule. The position requires a call rotation for nights, weekends, and holidays. What You'll Do: Consult with nursing staff and the multidisciplinary team regularly to evaluate the patient's status and appropriateness of care, including admission, length of stay, transfer, and discharge. Provides direct treatment and management of health conditions, including making recommendations to providers regarding additional services needed and referrals for community resources. Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Monitors, records, and communicates patient condition as appropriate. Participates in interdisciplinary team planning to collaborate with other disciplines to determine the appropriateness of care and other services needed. Provides discharge education to patients. Your Qualifications: A current license as a Registered Nurse in Maine or licensed from a compact state, required 1-2 years of acute or med/surg nursing experience, preferred Previous homecare, hospice, CCU, or ED experience, helpful Valid state driver's license with acceptable driving record, required Critical thinking skills, decisive judgment, and the ability to work with minimal supervision Strong technology/computer skills and previous experience with electronic medical records (EMR) preferred Scheduled Weekly Hours: 0 Work Shift: Job Exempt: No Benefits: Supporting all aspects of our employees’ wellness – physical, emotional and financial – is a critical component of being a great place to work.
