Description
Performs duties required to fulfill and respond to most release of information requests ensuring accuracy and completeness; complies with federal and state privacy laws according to Mendocino Community Health Clinic policies and procedures.
Primary Duties Include:
- Maintains current knowledge and interpretation of state and federal requirements relating to disclosure of protected health information, privacy and confidentiality in health information management.
- Demonstrates an understanding of the policies and procedures related to release of information, confidentiality, information security and information storage and retrieval.
- Analyzes and answers correspondence requests in compliance with applicable clinic, governmental policies and procedures and regulatory requirements.
- Processes and documents requests from ancillary departments and release of information (ROI) requests and incoming records through the electronic medical record (EMR); copies health records and demonstrates proficiency in EMR procedures.
- Prepares records for mailing: Copies appropriate portions of the records; charges appropriate fees; scans authorization forms into the patient’s health record. Accepts subpoenas, and reviews, screens and copies subpoenaed records; copies health records for external audits; assists all walk-ins with requests for their health records.
- Attends education/training for patient privacy and HIPAA compliance. Participates and promotes activities to support department compliance with PHI. Fosters information privacy awareness, patient privacy and HIPAA Attends to day-to-day workflow and function to minimize risk of protected health information breaches.

Requirements
- Ability to read, write and comprehend English
- Minimum of two years’ experience in Health Information Management, directly involved in the handling of PHI, electronic health records, and scanning.
Education:
- High School Diploma or General Education Degree (GED)
Other Preferred Qualifications:
- Bilingual, English/Spanish, fluent
- Two-year College Degree
- Certification in Health Information Management or Certified Medical Assistant with a Specialty in Administration
