Employment Type: Full Time | Monday - Friday, 8a-5p
Pay Rate: $61,000 / annually 3,000 Sign On
Location: Kanas City, KS
We are seeking a dedicated and compassionate Social Worker to join our interdisciplinary PACE team. Our Social Worker will support participants and their families through advocacy, counseling, care planning, and coordination of resources to enhance quality of life and independence. This role plays a critical part in helping older adults navigate complex social, emotional, and environmental challenges.
Midland Care’s Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
Key Responsibilities:
- Conduct psychosocial assessments and develop individualized care plans in collaboration with the interdisciplinary team.
- Facilitate family meetings, care conferences, and advanced care planning discussions.
- Participate in interdisciplinary team meetings, care planning, and continuous quality improvement initiatives.
- Advocate for participants’ rights and well-being at all times.

Qualifications
What we're looking for:
- Licensed in the state of Kansas as an LMSW.
- Minimum of 1–2 years of experience working with the elderly, frail adults, or individuals with complex care needs preferred.
- Knowledge of community resources, long-term care systems, and behavioral health principles.
- Valid driver’s license and ability to travel locally as needed
