Midland Care’s Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
Employment Type: Full Time | Monday - Friday, 8a-5p
Pay Rate: $65,600 / annually (pay is based on requirements and experience)
Location: Topeka, KS
Sign on Bonus: $3,000
We are seeking a dedicated Registered Nurse (RN) Case Manager to join our interdisciplinary team in the PACE program. In this vital role, you will oversee the delivery of high-quality clinical care and coordinate services to meet the individual needs of participants. The RN Case Manager functions as both a clinician and care coordinator, ensuring participants receive timely, effective, and compassionate support across all settings.
Key Responsibilities:
- Serve as the primary nursing contact and case manager for assigned PACE participants
- Monitor participant health status, intervene as appropriate, and coordinate care across home, clinic, and specialty settings
- Coordinate home care services, durable medical equipment, and specialist referrals

Qualifications
What we're looking for:
- Current Registered Nurse (RN) license in the state of Kansas
- Experience working with frail elderly or medically complex populations is strongly preferred
- Strong clinical judgment, organizational skills, and the ability to work effectively in a team-based environment
- Valid driver’s license and reliable transportation for occasional home visits
