As a key member of our team, you will receive, accept, and document new referrals while developing lasting relationships with clients and Direct Care Workers (DCWs).
Responsibilities include coordinating DCW assignments from start to completion, ensuring services align with our mission.

Key Responsibilities:
- Manage partnerships with Managed Care Plans, Case Managers, community partners, and referring entities for home-based services.
- Conduct intake assessments for new clients to review needs and expectations.
- Develop service plans addressing client needs according to company procedures.
- Ensure proper care and standards are met during ongoing home visits.
- Review and correct any deficiencies among direct care workers in a timely manner.
- Provide after-hours support and adjust scheduling changes as necessary.
- Investigate and research incidents and grievances.

Requirements:
- High school diploma or equivalent required.
- Minimum three years' experience in service coordination or delivery.
- Two years' supervisory experience required.
- Must be bilingual.

Salary : $60,000 - $81,000
