A Case Facilitator I – Adult operates under supervision of the assigned Area Director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to adult consumers.
First shift, M-F, 8 hours
High school diploma or equivalent.
Experience: Must meet OBHP requirements=Two years experience in mental health or community services preferred.
Salary based on experience.
State issued driver's license required.
