What are the responsibilities and job description for the Case Facilitator I - Child and Adolescent position at Northeastern Center Inc?
A Case Facilitator I - Child and Adolescent operates under supervision of the assigned Area Director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to child and adolescent consumers.
o Links, monitors, and refers clients to appropriate community resources
o Coordinates care with appropriate internal and external parties
o Ensures appropriate behavioral health services are provided.
High School diploma or equivalent.
Experience: Must meet OBHP requirements =Two years' experience in mental health or community services preferred.
State issued valid driver's license.
Salary based on experience.
