What are the responsibilities and job description for the Emergency Solutions Case Facilitator position at Northeastern Center Inc?
The Emergency Solutions Case Facilitator operates under supervision of the Emergency Services Team Manager. Primary job duties include on-call for crisis intervention services and acting as community liaison between NEC and referral sources. On call services include telephone and face-to-face interactions with clients. Follow-up action on previous emergency calls and maintaining positive working relationships with referral sources (hospitals, nursing homes, etc.)
Applicant must be willing to work nights, weekends, holidays, and fulfill on-call rotation.
