We are seeking a dedicated and collaborative team member to join our Community Rehabilitation & Treatment (CRT) Team. As a Case Manager, you will provide essential support to adults with severe mental illness, helping them lead more independent and fulfilling lives.
Key Responsibilities:
- Deliver services across various community-based settings, focusing on supportive counseling, symptom management, daily living activities, social and interpersonal skills development, money management, and advocacy.
- Maintain a positive attitude, work well within a team, and stay organized to manage caseloads effectively.
- Experience in mental health case management is a plus, but we are willing to train the right candidate.
Qualifications:
- Positive attitude, team-oriented, and strong organizational skills.
- Reliable transportation with the capacity to transport clients as needed.
- A bachelor’s degree is preferred, but an associate degree in human services or relevant experience will be considered.

Qualifications
Bachelor’s degree preferred. Willing to consider Associate’s Degree or equivalent combination of related education, training and experience.
