JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Utilize the Boston University Psychiatric Rehabilitation model to assist residents in rehabilitation goals and developing the skills and resources required to meet defined goals.
QUALIFICATIONS:
- Master's degree in the human services field.
- Experience working in a human service program.
- Good interpersonal skills.
- Driver's License required.
SPECIFIC DUTIES:
- Provide individual, group and family therapy at least weekly for all residents
- Ensure that delivery of clinical treatment is congruent with approved treatment models
- Be flexible in scheduling clinical treatment to meet resident and family needs
- Maintain on-going communication and collaboration with family members, to include scheduling, follow up and documentation of each therapeutic leave
- Complete biopsychosocial evaluations for potential admissions as assigned
- With the resident and treatment team, develop discharge plans and arrange resources defined in the discharge plan
- Complete all documentation required for continued stay and communicate authorizations to PATH’s QI department in a timely manner
- Participate in CBH phone reviews with program director or program manager as requested
- Complete individual service plans and individual treatment plans in a timely manner for all residents
- Participate in weekly treatment team meetings with the RTF Psychiatrist
- Provide psychiatric rehabilitation services for all residents
- Ensure the delivery of psychiatric rehabilitation services is congruent with the Boston University Model
- Assess decision making readiness regarding environment choices in living, learning, working and social areas
- Other duties as assigned.
PHYSICAL DEMANDS:
Minimal in nature but must be able to perform crisis management interventions including restraints once training is successfully completed.
