POSITION GOAL: The Assertive Community Treatment (ACT) team provides nontraditional community based mental health treatment to promote the recovery and community integration of adults with serious and persistent mental illnesses and cooccurring substance use and/or serious cooccurring physical health conditions. The overall goal of the ACT team is to reduce use of ER’s, inpatient hospital stays, and other harmful behaviors. The ACT Program Assistant is an integral part of a multidisciplinary team of mental health clinicians/providers. This position will provide support to the team, triage incoming calls, and day to day coordination of the program. The vast majority of services will take place in participant’s homes and/or in the community.

CPA 1: PROGRAM ASSISTANT COMPONENTS *
- Facilitates and coordinates the needs of persons receiving services with other team members.
- Provides support to the team as needed, which may include providing guidance on persons served situations, assisting with training, and/or providing support in the community with persons receiving services.
- Provides direct services to persons receiving services as needed, including but not limited to having an assigned caseload.
- Acts as a liaison to internal and external community partners.
- Maintains ACT phone to triage day to day situations with persons receiving services.
- Facilitates ongoing collaboration with community/government agencies to ensure appropriate services are provided to persons receiving services.
- Assists in monitoring program data and develops reports to ensure appropriate accountability.
- Facilitates other projects as requested.
CPA 2: ADMINISTRATIVE RESPONSIBILITIES *
- Accurately enter time through ADP on a daily basis.
- Maintain confidentiality in all aspects of position.
- Work with ACT Therapist on treatment planning and updating treatment plans.
- Communicate with community partners only after obtaining proper authorization.
- Complete paperwork tasks within required time frames.
- Attend and regularly participate in ACT team meetings.
- Attend and regularly participate in clinical and administrative supervision.
- Maintain responsibility for organizing a schedule and establishing priorities, according to existing patient and agency needs.

Maintain adequate and timely documentation in the clinical record of treatment interventions and other activities related to service participant care, as defined by Prairie View standards.
- Schedules are completed within 24 hours of the service provided.
- Progress notes are completed within 24 to 48 hours.
CPA 3: PRODUCTIVITY EXPECTATIONS *
- Encounter expectation is 24 to 27 encounters with persons receiving services per week.
- Ensure that contacts with service participants are goal directed, outcome focused, and meaningful.
CPA 4: AGE BASED COMPETENCY *
- Acknowledges the rights and responsibilities of adult service participants.
- Recognizes the range of needs, both mental and physical, based on an individual’s age.
- Proactively makes recommendations regarding specialized services, based on the individual’s age.
- Recognizes and differentiates typical adult behavior and atypical adult behavior.
- Is able to set and maintain appropriate boundaries when working with adults.
CPA 5: QUALITY ASSURANCE AND PERFORMANCE IMPROVEMENT (QAPI) *
- Participate in organizational QAPI activities. Be familiar with QAPI Initiatives and goals in work area. Annually review the Quality Assurance & Performance Improvement Plan.
CPA 6: CONFIDENTIALITY *
- Promote professionalism by discussing personal or service participant information in a private area.
CPA 7: PROFESSIONAL DEVELOPMENT *
- Participate as a member of Prairie View staff in structured meetings.
- Participate in continuing education activities and required inservices (such as Infection Control, etc.) as requested, to enhance and improve job performance.
- Utilize Relias Learning site to enhance knowledge in the areas of mental health and treatment modalities.

POSITION DESCRIPTION

POSITION QUALIFICATIONS:
- Minimum Education: Bachelor’s Degree, preferably in human services field, or equivalent
- Minimum Experience: Experience working with individuals experiencing severe and persistent mental illness preferred
- Must be at least 18 years of age
- Must have excellent communication skills listening, verbal, and written
- Must be able and willing to be flexible, a self starter, punctual, and have the ability to maintain a schedule
- Must have a valid Kansas driver’s license, be insurable through Prairie View insurance company, and be willing to provide transportation to patients as needed
- Must have ability to maintain professional boundaries with coworkers and persons receiving services
- Must be proficient at basic keyboarding/word processing skills
- Must be able to pass Kansas Bureau of Investigation Screen and the Central Registry Screen for both adults and children
