Summary:

The Team Scheduler performs administrative scheduling duties to ensure the smooth and efficient operation of the home health agency.

Essential Responsibilities and Duties: (Note: Other Duties May Be Assigned)

• Schedule initial Start of Care (SOC) visit once Clinical Manager has reviewed referral for appropriateness
• Obtain Case Conference Report from Clinical Manager once SOC is completed and schedule disciplines accordingly
• Maintain a professional and courteous demeanor when speaking with other Agency personnel, including field personnel
• As needed, answer telephones in a courteous manner and assist caller/direct to appropriate person
• Access past due visits report on a daily basis and contact clinicians via telephone when documentation is outstanding for greater than 72 hours
• Scanning documents in patient’s medical record as needed, including case conference/care coordination forms
• Schedule supervisory visits as per Agency’s policies and procedures

Qualifications: High School Diploma

Education and/or Experience:

• Working knowledge of office practices and procedures.
• Strong computer/data entry and software skills.
• Some medical terminology or Health care industry experience preferred.

Language Skills: Must possess good communication skills, both written and verbal in English. Ability to communicate in other foreign languages is desirable.

Mathematical Skills: Math skills required.

Reasoning Ability:

Able to work under pressure and handle multi-task functions. Must be able to deal effectively with people. Must be self-motivated and resourceful.

Physical Demands:

Physical demands for this position include an ability to be seated for extended periods of time and use computers/office equipment. Ability to lift light supplies and materials from time to time.
