Occupational Therapist Part Time for Home Health

Occupational Therapist Job Summary:

Deer Meadows Home Health is seeking a dedicated and compassionate Occupational Therapist to join our Home Health team on a part-time basis. As an integral member of our interdisciplinary team, you will play a vital role in helping patients recover and achieve their maximum potential in the comfort of their own homes.

Occupational Therapist Responsibilities:

• Conduct thorough assessments of patients' physical and cognitive abilities, identifying areas of strength and weakness
• Develop and implement individualized occupational therapy plans to address patients' specific needs and goals
• Collaborate with patients, families, and healthcare providers to ensure seamless transitions and optimal outcomes
• Provide education and training to patients and caregivers on techniques and strategies for improving daily functioning and independence
• Document patient progress and maintain accurate records
• Participate in case conferences and team meetings to ensure comprehensive care
  
  
  

Occupational Therapist Requirements:

• Current licensure as an Occupational Therapist in the state of Pennsylvania
• Prior experience in treating in Home Health
• Ability to travel to patient homes and work independently
• Must have own reliable transportation; travel reimbursements provided
• Strong communication and interpersonal skills
• Compassionate and patient-centered approach to care
