You are an RN who:
- Values building connections with their patients.
- Likes working independently while backed by a collaborative team.
- Has great written and oral communication
- Hold or can get a current License/certification in Wisconsin.
- Home health experience preferred.
Job Duties Include:
- Serve as a Case Manager, coordinating patient care and services.
- Conduct thorough patient assessments and develop individualized care plans.
- Collaborate with patients, families, and the clinical team to achieve optimal outcomes.
- Document care and treatment plans clearly and concisely, including discharge summaries.
- Educate patients and caregivers on safe discharge plans and follow-up care.
