Description
- Responsible for advocacy and providing assistance to clients in obtaining permanent housing.
- Initiate housing referrals to the appropriate agencies.
- Meet with clients on a regular basis to develop strategies to meet their housing needs.
- Coordinate permanent housing search and address any barriers that may arise.
- Coordinate and facilitate quarterly Housing Forums for landlords, brokers and clients.
- Coordinate and facilitate bi-weekly meetings with clients.
- Conduct housing workshops for housing readiness.
- Escort clients to apartment viewings to meet prospective landlords and real estate brokers.
- Participate and attend the Interdisciplinary Team and Administrative bi-weekly meetings.
- Submit all monthly reporting in a timely manner. Ensure documentation is reflected in the clients charts.
- Initiate and identify housing resources for street and stabilization bed clients, and facilitate applications on their behalf (i.e. 2010Es, Supportive Housing, and other appropriate housing applications.
- Coordinate and case conference with both internal and external programs to ensure care coordination as it relates to housing.
- Attend appropriate DHS trainings and meetings
- Maintain a caseload of Retention clients to ensure clients maintain their housing placement.
- Complete Monthly notes for clients in AWARDS and DHS database.
Qualifications
This position requires a Bachelors Degree and/or previous housing experience working with the homeless, mentally ill, and chemically addicted populations. Knowledge of housing resources and entitlements required. Must have exceptional communication skills both orally and in writing with individuals and groups regarding complex and sensitive issues. Physical ability to perform the essential job functions are needed. In addition, the Housing Specialist shall perform any duties as required to meet the needs of the program and the clients we serve. Salary : $38,000
