About The Role
- Be on-call from Saturday at 8:00 AM through Monday at 8:00 AM
- Respond to patient calls and care concerns over the phone
- Provide in-person field visits when necessary
- Collaborate with the care team to assess and resolve urgent home health needs
- Accurately document triage decisions and patient visits
- $700 flat stipend for the full on-call weekend
- $25–$30/hour for any time spent on visits (DOE)
- Mileage reimbursement for patient visits
- Work from home when not in the field!
- Be part of a respected non-profit organization focused on quality and compassion
- Active LPN license in the State of Colorado
- Home health experience preferred
- Excellent judgment and problem-solving under pressure
- Ability to prioritize and communicate effectively
- Reliable transportation, valid driver's license, and insurance
- Comfortable working independently in a home health setting
