What You'll Do
- Provide hands-on nursing care and health education in the home setting
- Monitor patient conditions and report changes to the RN or clinical team
- Perform wound care, medication administration, catheter care, and more
- Support chronic disease management and patient safety in collaboration with families and caregivers
- Accurately document visits using our EMR system
- Travel locally within Mesa County for patient visits (mileage reimbursed)
- Competitive pay: $25–$30/hour depending on experience
- Flexible Schedule - Monday–Friday, 8 AM – 5 PM
